1994-1995 National Health Interview Survey (NHIS) Disability Phase II Polio Public Use Data File WARNING - DATA USE RESTRICTIONS! Read Carefully before Using The Public Health Service Act (Section 308 (d)) provides that the data collected by the National Center for Health Statistics (NCHS), Centers for Disease Control and Prevention (CDC), may be used only for the purpose of health statistical reporting and analysis. Any effort to determine the identity of any reported case is prohibited by this law. NCHS does all it can to assure that the identity of data subjects cannot be disclosed. All direct identifiers, as well as any characteristics that might lead to identification, are omitted from the data set. By using these data, you signify your agreement to comply with the above-stated statutorily based requirements. * * * * * * * * * * * * * * * * * * * * * * * * * * * * * * * * * * * * * * * Introduction In 1994 and 1995, the National Health Interview Survey (NHIS) conducted a Disability Followback Survey (DFS) based on responses to a Disability Phase 1 Supplement. There were four DFS questionnaires: one for adults with a past history of Polio; one for children and one for adults with an indication disability; and one for elderly persons 69 years of age and older without any indication of disability, called the Supplement on Aging or SOA. The Polio, DFS-Child, and DFS-Adult questionnaires were fielded in 1994 and 1995. The Polio Questionnaire collected extensive information about the symptoms and impact of the illness at the time of initial diagnosis of polio and at various times after the diagnosis. This includes questions on periods of rehabilitation, post-polio syndrome, times when respondents felt they were at their "physical best", and other health problems experienced. Whenever possible, self response by the polio survivor was required for this component. Users should note that because of the small sample sizes, many single year estimates will not be reliable and variance estimators will be unstable. It is recommended that separate year analyses not be done. Thus, description of several important aspects of the file are found in the file documentation. Data users should use great care in reading all preliminary notes and information in the file documentation. Overview of the 1994-95 NHIS Polio Data File The 1994-1995 NHIS Polio public use data file contains 566 records with a record length of 670. Each record contains data for one person. The contents of the file are described in detail in the file documentation which includes unweighted frequencies for each variable. In general, the 1994-1995 NHIS Polio file is organized in the following manner: a. The NHIS person record from the core questionnaire (locations 1-189); b. The weight and design fields (locations 190-388); c. Polio data fields (locations 389-670); The 1994-1995 NHIS Polio Outcome Data File The Polio Outcome File identifies response or non-response for each adult identified as eligible for the Polio questionnaire in Phase II, and provides a code which indicates the type of outcome that resulted from following-up with eligible respondents. For ease of transmission, the 1994-1995 NHIS Polio Outcome Data file has been converted into a self-extracting compressed format. To load and expand a compressed file into an ASCII file, perform the following steps: Download the compressed file onto your hard drive into an appropriate subdirectory. For example: Copy the data file OUTCOMEP.EXE into a subdirectory called POLIO. To extract OUTCOMEP.EXE into the ASCII file OUTCOMEP.DAT, go to the subdirectory POLIO and do the following: - From DOS - type OUTCOMEP without the EXE extension. - From Windows - Double-click on OUTCOMEP.EXE.
