                                             survey.  Includes background on the survey
                                             mechanism and the sample, data collection
                                             procedures, data editing, and estimation.
                                             Also includes guidelines for use of the data
                                             set.  PDF format.

asthma_4state.zip -------------------------- Compressed zip file containing the asthma four state public use data set.  The 
                                             uncompressed file is a SAS version 8 data file about 30 megabytes 
                                             in size unzipped.
                                             
asthma_national.zip ------------------------ Compressed zip file containing the asthma national public use data set.  The
                                             uncompressed file is a SAS version 8 data file about 50 megabytes in size 
                                             unzipped.

create_formatted_frequencies_national.sas--- Sample SAS program that uses the national_master_formats.sas
                                             program and the assign_asthma_national_formats.sas program to create and attach 
                                             formats.  This program creates a permanent format library and a
                                             permanent SAS data set with the formats attached.  This program also
                                             produces the output contained in the contents_national.pdf and the 
                                             frequencies_national.pdf. SAS program file.
